There is some confusion in describing element, for example

in "3 Simultaneous ID and NAME attributes"
... provided for the <A> [W3C 12.2] and <MAP> [W3C 13.6.1] elements.
in "6 FORM content model"
The BUTTON element should ... of a FORM element.

They should be described in an identical style, e.g., <XXX> element.
